• Copy of the Certificate obtained under the Cooperative Act, 1861, wherever applicable.
• Form IX – Nomination of Authorised Person.

Additional Documents for Specific Food Businesses

• Hotels, Restaurants, Food Vending Establishments, Clubs & Canteens: Analysis report of water used as an ingredient in food from a recognised/public health laboratory.
• Importers: Import Export Code (IEC) and Recall Plan.
• Merchant Exporters: Import Export Code (IEC) along with a declaration stating that products meant for export will not be sold in the domestic market.
• Head Office / Registered Office / E-Commerce Operators:Recall Plan.
• Transporters: List of Vehicle Registration Numbers.

Documents Required for Manufacturers and Processors

Food manufacturers and processors are required to submit the following additional documents while applying for an FSSAI License.

• List of Directors, Partners, Proprietor, or Executive Members with complete contact details.
• Government-issued Identity Proof and Address Proof of all authorised persons.
• Proof of possession of the business premises.
• Partnership Deed, Proprietorship Declaration, MOA, AOA, or other constitutional documents.
• Certificate obtained under the Cooperative Act, 1861 (where applicable).
• Form IX – Nomination of Authorised Person.
• Blueprint or layout plan of the processing unit showing dimensions and operation-wise area allocation.
• Photographs of the production unit.
• List of machinery and equipment with installed capacity, horsepower, and quantity.
• Chemical and bacteriological analysis report of water used as a food ingredient from a recognised/public health laboratory.
• Additional supporting documents and prescribed government fees may also be required depending on the business category.

Validity and Renewal of FSSAI License

Every FSSAI License or Registration is issued for a specified validity period selected by the Food Business Operator at the time of application.

• FSSAI Licenses are issued with a validity ranging from 1 year to 5 years.
• The validity period begins from the date of issue of the License or Registration.
• Renewal should be initiated at least 30 days before the expiry of the existing License or Registration.
